Important Due Dates

It can be difficult to remember due dates. Below is a list of important due dates.
Medical / Dental Forms

  • Physician's Report - Within 30 days
  • CFS 125 Medical or Dental Appointment - 48 Hours
  • Hospital Discharge Paperwork - As soon as possible
  • Medication Admin. Record (MAR) - 10th of every month

Activity / Allowance

  • Allowance/Activity Form - 10th of every month
  • Clothing Receipts- 10th of every month
  • Clothing Inventory - Every 3 months
  • Report Cards - Every Semester/Quarter
